It seems that i have tried every possible configuration except that one. It now records both parties perfectly (Voice Call + AAC High Quality ) is the way to go.
Now, i'll just need to wait until tomorrow to see if i can get the log sent to you. Note that i need to wait until tomorrow morning, as it works fine through out the day...

I haven't done it before because i realized that it started working every morning without problems. With that in mind i figured that you wouldn't have much to see though, it just stopped working again so.. please have a look when you get a chance.

I'm glad I found this thread. My Samsung Note 4 (SM-N910G) initially worked fine with my script, which started the recording immediately upon being triggered by an incoming or outgoing call. I actually split it into two scripts - one for incoming and the other for outgoing - because of the file naming convention I chose to use in each situation.
Anyway, ever since I installed a profile switching app ("Profile Scheduler") for managing all of my various volumes in one fell swoop, I could only get full level of the incoming audio on the incoming calls script but not the outgoing. That's very weird because both have always been set to high quality AAC and Voice Calls for the audio source. Basically, the action is identical for both apart from the file naming convention.
Anyway, once I inserted the 2s sleep action which you proposed after the outgoing call trigger, the issue was resolved so thank you for the tip. Any idea why the 2s sleep action is so crucial? Is it something that happens during the set up of the call by Samsung's firmware which Profile Scheduler possibly messed with?

I'm not sure why the sleep action helps in the case of the outgoing call. I just noticed that the triggers for incoming and outgoing calls are often quite fast so that the device might still be in the process of applying the audio routing for the call. Maybe the profile scheduler app introduced an additional small delay since it probably also changes some audio related properties right at the same time.
